Forms & Strengths
- Oral powder, various strengths
- Oral suspension, various concentrations
- Combination antacid chewable tablets
Adult Dosing
- Dyspepsia/Antacid: 1 to 2 g orally as needed
- Maximum daily dose typically not to exceed 10 g due to laxative and systemic absorption risks
Pediatric Dosing
- Not routinely recommended in pediatric patients
- Safety and efficacy for pediatric antacid use not well established
Indications
- Symptomatic relief of hyperacidity, heartburn, and dyspepsia associated with peptic ulcer disease or gastritis
Mechanism of Action
- Acts as an antacid by neutralizing gastric hydrochloric acid, raising gastric pH
- Provides a protective coating over the gastric mucosa
Contraindications
- Severe renal impairment (risk of hypermagnesemia)
- Hypersensitivity to magnesium silicates
- Fecal impaction or intestinal obstruction
- Undiagnosed abdominal pain
Adverse Reactions
- Diarrhea (due to unabsorbed magnesium salts)
- Nausea and abdominal discomfort
- Hypermagnesemia in patients with renal impairment
- Electrolyte disturbances with chronic high-dose use
Drug Interactions
- Decreases absorption of tetracyclines, fluoroquinolones, iron, and ketoconazole via chelation and increased gastric pH
- May alter the excretion of acidic or basic drugs by changing urinary pH
- May enhance or interfere with the effects of other antacids or laxatives
