Forms & Strengths
- Oral Suspension: Magaldrate 400-540 mg / Simethicone 20-40 mg per 5 mL
- Chewable Tablets: Magaldrate 480 mg / Simethicone 20 mg
Adult Dosing
- 5 to 10 mL (or 1 to 2 tablets) between meals and at bedtime, or as needed for symptoms
- Maximum daily dose: Follow package labeling or clinician guidance, typically not to exceed QID dosing
Pediatric Dosing
- Safety and efficacy in pediatric patients under 12 years are not well established
- Use in children is generally discouraged unless specifically directed by a pediatrician
Indications
- Symptomatic relief of hyperacidity associated with peptic ulcer disease, gastritis, and esophagitis
- Relief of symptoms associated with gas, including flatulence, bloating, and postprandial discomfort
Mechanism of Action
- Magaldrate is a hydromerate of magnesium and aluminumates that rapidly neutralizes gastric hydrochloric acid
- Simethicone is a surface-active antiflatulent agent that coalesces gas bubbles in the gastrointestinal tract to facilitate elimination
Contraindications
- Hypersensitivity to aluminum or magnesium salts, or any component of the formulation
- Severe renal impairment or failure due to risk of aluminum and magnesium accumulation and toxicity
- Magnesium-induced laxative effect worsening severe diarrhea or bowel obstruction
Adverse Reactions
- Constipation (primarily due to aluminum)
- Diarrhea (primarily due to magnesium)
- Hypermagnesemia in patients with renal dysfunction
- Hypophosphatemia with prolonged use of aluminum-containing antacids
Drug Interactions
- Decreases absorption of concurrently administered oral medications such as tetracyclines, fluoroquinolones, iron, and digoxin via chelation or increased gastric pH
- Separate administration of interacting drugs by at least 2 hours before or after magaldrate
