sodium phosphate
A06AG01
Forms & Strengths
- Enema: Monobasic sodium phosphate 19 g and dibasic sodium phosphate 7 g per 118 mL delivered volume
- Oral solution: Monobasic sodium phosphate 48 g and dibasic sodium phosphate 18 g per 45 mL
Adult Dosing
- Constipation/Bowel cleansing: One delivered enema (118 mL) rectally as a single dose
- Colonoscopy preparation (Oral): 45 mL aqueous solution diluted in 4 ounces of clear liquids, followed by 8 ounces of clear liquids, repeated once (total 90 mL) as a split-dose regimen
Pediatric Dosing
- Children 2 to 11 years: Rectal enema 59 mL (half-bottle) as a single dose, use with caution
- Children under 2 years: Contraindicated for rectal enema use
- Children under 18 years (Oral solution): Safety and efficacy not established, not recommended for bowel preparation
Indications
- Treatment of occasional constipation
- Bowel cleansing prior to colonoscopy, sigmoidoscopy, or radiological procedures
Mechanism of Action
- Acts as an osmotic laxative in the intestinal tract
- Draws water into the lumen of the colon via osmotic gradient
- Increases intraluminal pressure and stimulates peristalsis
Contraindications
- Congestive heart failure
- Clinically significant renal impairment
- Ascites
- Suspected bowel obstruction, ileus, or perforation
- Acute surgical abdomen or inflammatory bowel disease flares
- Known hypersensitivity to sodium phosphate products
Adverse Reactions
- Hyperphosphatemia
- Hypocalcemia
- Hypokalemia
- Dehydration and electrolyte imbalances
- Acute phosphate nephropathy (renal failure)
- Abdominal pain, cramping, bloating, nausea, and vomiting
- Rectal irritation or ulceration (with enema use)
Drug Interactions
- Drugs that may affect electrolyte levels (diuretics, ACE inhibitors, ARBs, NSAIDs) increasing risk of nephrocalcinosis
- Oral medications with absorption affected by rapid GI transit (may be flushed from the GI tract)
- Calcium channel blockers and other agents that impair renal function
