Forms & Strengths
- Injection: 100 units/mL (U-100) in 10 mL vials
Adult Dosing
- Subcutaneous: Individualized based on blood glucose monitoring, metabolic needs, and meal patterns
- Type 1 Diabetes: Typically 0.2 to 0.6 units/kg/day divided into multiple daily doses (prandial and basal)
- Intravenous: Regular pork insulin used in acute clinical settings (e.g., DKA) under strict protocols
Pediatric Dosing
- Subcutaneous: Individualized based on weight, age, and pubertal status
- Initial total daily dose: Typically 0.2 to 0.5 units/kg/day, adjusted based on glycemic control
Indications
- Glycemic control in patients with type 1 diabetes mellitus
- Glycemic control in patients with type 2 diabetes mellitus requiring insulin therapy
- Management of diabetic ketoacidosis or hyperosmolar hyperglycemic state (via IV infusion)
Mechanism of Action
- Binds to insulin receptors on muscle and fat cells, facilitating cellular uptake of glucose
- Inhibits hepatic gluconeogenesis and glycogenolysis
- Promotes anabolic processes including lipogenesis and protein synthesis while inhibiting lipolysis
Contraindications
- Hypoglycemia
- Hypersensitivity to pork insulin or any excipients in the formulation
Adverse Reactions
- Hypoglycemia (common, potentially severe)
- Weight gain
- Hypokalemia
- Injection site reactions (lipodystrophy, lipoatrophy, pruritus, erythema)
- Allergic reactions, including anaphylaxis and generalized skin rashes
Drug Interactions
- Glucose-lowering agents (sulfonylureas, GLP-1 receptor agonists, SGLT2 inhibitors): May increase hypoglycemia risk
- Corticosteroids, thiazide diuretics, sympathomimetics, atypical antipsychotics: May decrease blood glucose-lowering effect
- Beta-blockers: May mask symptoms of hypoglycemia (e.g., tachycardia, tremors) and prolong hypoglycemic episodes
- Alcohol: Can potentiate hypoglycemia or cause delayed hyperglycemia depending on consumption patterns
