glimepiride and dapagliflozin
A10BD32
Forms & Strengths
- Fixed-dose combination tablet: Glimepiride 2 mg / Dapagliflozin 10 mg
- Fixed-dose combination tablet: Glimepiride 4 mg / Dapagliflozin 10 mg
Adult Dosing
- Type 2 diabetes mellitus: One tablet daily taken in the morning with or without food
- Max daily dose: Glimepiride 4 mg / Dapagliflozin 10 mg daily
- Renal impairment: Contraindicated if eGFR < 45 mL/min/1.73m²
Pediatric Dosing
- Safety and efficacy not established in pediatric patients under 18 years of age
Indications
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both glimepiride and dapagliflozin is appropriate
Mechanism of Action
- Glimepiride: Sulfonylurea that stimulates insulin release from pancreatic beta cells and reduces glucose output from the liver
- Dapagliflozin: Sodium-glucose cotransporter 2 (SGLT2) inhibitor that inhibits renal reabsorption of glucose, increasing urinary glucose excretion and lowering blood glucose
Contraindications
- Hypersensitivity to glimepiride, dapagliflozin, or sulfonamide derivatives
- Type 1 diabetes mellitus or diabetic ketoacidosis
- Severe renal impairment (eGFR < 45 mL/min/1.73m²), end-stage renal disease, or patients on dialysis
Adverse Reactions
- Hypoglycemia
- Female genital mycotic infections
- Nasopharyngitis
- Urinary tract infections
- Back pain
- Polyuria
- Volume depletion and hypotension
Drug Interactions
- Insulin or insulin secretagogues (e.g., sulfonylureas): Increased risk of severe hypoglycemia
- CYP2C9 inhibitors/inducers: May alter glimepiride metabolism and plasma concentrations
- Diuretics: Dapagliflozin may additive-enhance diuretic effect and increase risk of dehydration and hypotension
- Lithium: SGLT2 inhibitors may increase renal lithium clearance, leading to decreased serum lithium concentrations
Curated Content: Needs Vetting Before Put to Clinical Use
