vildagliptin
A10BH02
Adult Dosing
- Type 2 Diabetes Mellitus: 50 mg once daily in the morning or 50 mg twice daily (morning and evening)
- Maximum daily dose: 100 mg
- When used in combination with a sulfonylurea, the recommended dose is 50 mg once daily in the morning
Pediatric Dosing
- Safety and efficacy in pediatric patients (< 18 years) have not been established
Indications
- Type 2 diabetes mellitus to improve glycemic control as monotherapy or in combination with metformin, a sulfonylurea, or a thiazolidinedione when diet and exercise are inadequate
Mechanism of Action
- Dipeptidyl peptidase-4 (DPP-4) inhibitor
- Prevents the inactivation of incretin hormones GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide) by DPP-4
- Increases insulin secretion in a glucose-dependent manner and decreases glucagon secretion, lowering blood glucose
Contraindications
- Hypersensitivity to vildagliptin or any excipients
- Type 1 diabetes mellitus
- Diabetic ketoacidosis
- Severe hepatic impairment
Adverse Reactions
- Common: Tremor, headache, dizziness, nasopharyngitis, upper respiratory tract infection
- Serious: Hepatic dysfunction/hepatitis, acute pancreatitis, bullous pemphigoid, hypoglycemia (especially when combined with a sulfonylurea or insulin)
Drug Interactions
- Thiazides, corticosteroids, thyroid products, and sympathomimetics may reduce the glucose-lowering effect of vildagliptin
- ACE inhibitors may increase the risk of angioedema
- Minimal potential for pharmacokinetic drug-interactions as vildagliptin is not a substrate, inhibitor, or inducer of CYP450 enzymes
