sotagliflozin
A10BK06
Forms & Strengths
- Oral tablets: 200 mg
- Oral tablets: 395 mg
Adult Dosing
- Heart failure, type 2 diabetes mellitus, chronic kidney disease: 200 mg orally once daily before the first meal of the day
- May be increased to 395 mg orally once daily based on tolerability and clinical response
- Renal impairment: Not recommended if eGFR < 25 mL/min/1.73m2
Pediatric Dosing
- Safety and efficacy in pediatric patients (< 18 years) have not been established
Indications
- Reduce the risk of cardiovascular death, hospitalization for heart failure, and urgent heart failure visit in adults with heart failure or type 2 diabetes mellitus, chronic kidney disease, and other cardiovascular risk factors
Mechanism of Action
- Dual inhibitor of sodium-glucose cotransporter 2 (SGLT2) and sodium-glucose cotransporter 1 (SGLT1)
- Inhibition of SGLT2 in the proximal renal tubule reduces renal glucose and sodium reabsorption, increasing urinary glucose excretion and osmotic diuresis
- Inhibition of SGLT1 in the intestinal tract delays and decreases glucose and galactose absorption
Contraindications
- History of serious hypersensitivity reaction to sotagliflozin
- Patients on dialysis
Adverse Reactions
- Volume depletion and hypotension
- Urinary tract infections
- Genital mycotic infections
- Diarrhea
- Diabetic ketoacidosis in patients with type 1 or type 2 diabetes
- Hypoglycemia (when used with insulin or insulin secretagogues)
- Increased LDL cholesterol
Drug Interactions
- Diuretics: May additive hypotension and volume depletion
- Insulin and insulin secretagogues: Increased risk of hypoglycemia
- Lithium: SGLT2 inhibitors may increase renal lithium clearance, decreasing serum lithium concentration
- Digoxin: Monitor concentrations closely as tubular secretion may be affected
Curated Content: Needs Vetting Before Put to Clinical Use
