Forms & Strengths
- Extended-release capsules: 30 mcg
Adult Dosing
- Secondary hyperparathyroidism in stage 3 or 4 chronic kidney disease: 30 mcg orally once daily
- Adjust dose at 4-week intervals based on serum total 25-hydroxyvitamin D, intact PTH, serum calcium, and serum phosphorus levels
Pediatric Dosing
- Safety and efficacy not established in pediatric patients
Indications
- Treatment of secondary hyperparathyroidism in adult patients with stage 3 or 4 chronic kidney disease and serum total 25-hydroxyvitamin D levels < 30 ng/mL
Mechanism of Action
- Prohormone of calcitriol (1,25-dihydroxyvitamin D3), the active form of vitamin D
- Binds to vitamin D receptors in the parathyroid glands to suppress PTH synthesis and secretion
- Promotes intestinal calcium and phosphorus absorption and bone mineralization
Contraindications
- Hypercalcemia
- Vitamin D toxicity
- Known hypersensitivity to calcifediol or any excipients
Adverse Reactions
- Nasopharyngitis
- Increased blood creatinine
- Urinary tract infection
- Hypercalcemia
- Vomiting
- Constipation
- Back pain
Drug Interactions
- Thiazide diuretics may increase the risk of hypercalcemia
- CYP3A4 inhibitors may increase calcifediol exposure and risk of toxicity
- CYP3A4 inducers may decrease calcifediol efficacy
- Magnesium-containing antacids/laxatives may cause hypermagnesemia in chronic renal failure patients
