Forms & Strengths
- Oral solution: 15 mg/mL (as d-alpha-tocopheryl polyethylene glycol 1000 succinate, TPGS)
Adult Dosing
- Vitamin E deficiency secondary to malabsorption: 15 to 25 IU/kg/day orally
Pediatric Dosing
- Pediatric vitamin E deficiency due to congenital chronic cholestatic hepatobiliary disease: 15 to 25 IU/kg/day orally
- Neonates/Infants requiring high-potency water-soluble vitamin E supplementation: individualized based on serum alpha-tocopherol levels
Indications
- Treatment and prevention of vitamin E deficiency associated with malabsorption syndromes, such as congenital chronic cholestatic hepatobiliary disease
Mechanism of Action
- Water-soluble derivative of natural alpha-tocopherol that acts as a lipid-soluble antioxidant in cell membranes
- Facilitates dietary vitamin E absorption in patients with lipid malabsorption and bile salt deficiency by forming water-miscible micelles
Contraindications
- Hypersensitivity to tocofersolan or vitamin E derivatives
- Premature infants (due to excipient polyethylene glycol toxicity risks, specifically benzyl alcohol or polysorbate toxicities if applicable to specific formulations, though TPGS itself carries low risk)
Adverse Reactions
- Nausea
- Diarrhea
- Abdominal pain
- Fatigue
- Headache
Drug Interactions
- Anticoagulants and antiplatelet agents (increased risk of bleeding due to vitamin E inhibition of platelet function at high doses)
- Iron supplements (concurrent high-dose vitamin E may impair hematological response to iron therapy in iron-deficiency anemia)
