ALIMENTARY TRACT AND METABOLISM › ANABOLIC AGENTS FOR SYSTEMIC USE › ANABOLIC STEROIDS › Androstan derivatives
norethandrolone
A14AA09
Adult Dosing
- Anabolic therapy: 10 to 30 mg daily orally, adjusted based on clinical response and tolerability
- Max duration: Typically limited to 6-12 weeks to minimize hepatotoxicity
Pediatric Dosing
- Safety and efficacy not established in pediatric patients; generally avoided due to risk of premature epiphyseal closure
Indications
- Adjunctive therapy for conditions requiring nitrogen balance improvement
- Severe catabolic states, chronic debilitating illnesses, and recovery from major surgery or trauma
Mechanism of Action
- Synthetic 17-alpha-alkylated anabolic-androgenic steroid derived from nandrolone
- Binds to intracellular androgen receptors in target tissues, promoting protein synthesis and decreasing amino acid excretion
Contraindications
- Known or suspected carcinoma of the prostate or breast in males
- Pregnancy (Category X due to virilization potential of the female fetus)
- Nephrosis or the nephrotic phase of nephritis
- Severe hepatic impairment or active liver disease
- Breast cancer in females with hypercalcemia
Adverse Reactions
- Hepatotoxicity including cholestatic jaundice, peliosis hepatis, and hepatocellular neoplasms
- Fluid retention, edema, and electrolyte disturbances (sodium, potassium, chloride, calcium)
- Virilization in females (deepening of voice, hirsutism, acne, clitoral enlargement)
- Testicular atrophy, oligospermia, and gynecomastia in males
- Alterations in lipid profile (decreased HDL, increased LDL)
- Premature closure of epiphyses in pediatric patients
Drug Interactions
- Anticoagulants: Potentiates effects of oral anticoagulants, increasing bleeding risk; monitor INR closely
- Insulin and oral antidiabetic agents: Anabolic steroids may decrease blood glucose and insulin requirements
- Corticosteroids: Concurrent use may increase the likelihood of severe edema
- Hepatotoxic drugs: Increased risk of hepatotoxicity when co-administered with other liver-toxic agents
Curated Content: Needs Vetting Before Put to Clinical Use
