Forms & Strengths
- Injection: 750 anti-Xa units per 0.6 mL in ampoules
Adult Dosing
- Prophylaxis of DVT: 750 anti-Xa units subcutaneously twice daily
- Treatment of DVT/PE: Weight-adjusted intravenous bolus followed by continuous infusion (e.g., 1250 to 2500 units IV, then maintenance infusion based on anti-Xa levels)
- Hemodialysis anticoagulation: Initial bolus followed by maintenance doses based on dialysis duration and anti-Xa monitoring
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Indications
- Prophylaxis of deep vein thrombosis (DVT) in patients undergoing elective hip replacement surgery
- Treatment of acute DVT and pulmonary embolism
- Anticoagulation in patients with heparin-induced thrombocytopenia (HIT) requiring alternative antithrombotic therapy
Mechanism of Action
- Glycosaminoglycan mixture (heparan sulfate, dermatan sulfate, chondroitin sulfate) that inhibits factor Xa predominantly via antithrombin III, and thrombin to a lesser extent
Contraindications
- Known hypersensitivity to danaparoid or pork products
- Active major bleeding or conditions with high risk of hemorrhage
- History of immune thrombocytopenia induced by danaparoid
- Severe hemorrhagic diathesis such as hemophilia or severe thrombocytopenia
Adverse Reactions
- Bleeding and hemorrhage at various sites
- Injection site hematoma, pain, or irritation
- Thrombocytopenia (cross-reactivity with HIT antibodies is low but possible)
- Elevated liver enzymes
- Skin rash and allergic reactions
Drug Interactions
- Increased bleeding risk with concomitant use of oral anticoagulants (e.g., warfarin)
- Increased bleeding risk with antiplatelet agents (e.g., aspirin, NSAIDs, clopidogrel)
- Increased risk of hemorrhage with thrombolytic agents
- Pharmacodynamic potentiation by other parenteral anticoagulants
