sulodexide
B01AB11
Forms & Strengths
- Oral capsules: 250 LSU (Lipasemic Sulfoxide Unit)
- Oral capsules: 500 LSU
- Solution for injection: 60 LSU/2 mL
Adult Dosing
- Diabetic nephropathy / microalbuminuria: 500 LSU orally twice daily
- Peripheral artery disease / intermittent claudication: 600 LSU (1 ampoule) intramuscularly or intravenously daily for 20 days, followed by 250 LSU orally twice daily for 2 months
- Venous leg ulcers: 500 LSU orally twice daily as an adjunct to standard therapy
Pediatric Dosing
- Safety and efficacy not established in patients under 18 years of age
Indications
- Management of vascular diseases with a thrombogenic risk
- Treatment of diabetic nephropathy with persistent microalbuminuria
- Intermittent claudication secondary to peripheral arterial disease
- Venous leg ulcers as an adjunct to compression therapy
Mechanism of Action
- Purified glycosaminoglycan composed of fast-moving heparin (80%) and dermatan sulfate (20%)
- Exhibits antithrombotic and profibrinolytic effects via affinity for antithrombin III and heparin cofactor II
- Protects vascular endothelial cells and restores the structural integrity of the endothelial glycocalyx
- Reduces blood viscosity and systemic inflammation markers
Contraindications
- Known hypersensitivity to sulodexide, heparin, or heparinoid compounds
- Active hemorrhagic diathesis and bleeding disorders
- Patients receiving ongoing treatment with heparin or oral anticoagulants (risk of enhanced anticoagulant effect)
Adverse Reactions
- Epigastric pain, nausea, vomiting, and diarrhea
- Dizziness and headache
- Skin rashes, urticaria, and injection site hematoma
- Increased risk of bleeding in predisposed patients
Drug Interactions
- Anticoagulants (heparin, oral anticoagulants): concomitant use may enhance anticoagulant effects and increase bleeding risk
- Antiplatelet agents (aspirin, clopidogrel): potential synergistic increase in bleeding risk
