Forms & Strengths
- intravenous solution 100 mg/mL
- single-use vials 50 mg, 100 mg, 125 mg, 250 mg
Adult Dosing
- initial dose 2 mcg/kg/min, adjust to achieve aPTT 1.5-3 times baseline
- renal dose adjustment not necessary
- target aPTT 45-75 seconds for PCI
Pediatric Dosing
- neonates, infants, children: limited data available, use with caution
- adolescents: dosing similar to adults, adjust to achieve target aPTT
Indications
- treatment or prophylaxis of HIT
- anticoagulation in patients with HIT undergoing PCI
Mechanism of Action
- direct thrombin inhibitor, binds and inactivates thrombin
- does not require antithrombin III for activity
Contraindications
- active major bleeding
- history of hypersensitivity reaction to argatroban
Adverse Reactions
- bleeding, hematoma, thrombocytopenia
- hypersensitivity reaction, anaphylaxis
Drug Interactions
- increased risk of bleeding with concomitant anticoagulants, antiplatelets
- reduced efficacy with concomitant unfractionated heparin
