Forms & Strengths
- Topical powder for solution: 1,000 IU
- Topical powder for solution: 5,000 IU
- Topical powder for solution: 10,000 IU
- Topical powder for solution: 20,000 IU
- Topical solution: 1,000 IU/mL
Adult Dosing
- Topical application: 1,000 IU to 2,000 IU per site depending on bleeding severity
- Can be used in conjunction with a sterile gelatin sponge
- Apply directly to the bleeding surface as a solution or powder
Pediatric Dosing
- Topical application: Safety and efficacy not formally established in neonates
- Use lowest effective volume and concentration based on body surface area in children
Indications
- Aid to hemostasis in procedures involving capillary and small venule bleeding
- Adjunct in surgical procedures where standard hemostatic techniques are ineffective or impractical
Mechanism of Action
- Catalyzes the conversion of fibrinogen to fibrin
- Promotes localized blood clot formation at the site of topical application
Contraindications
- Known hypersensitivity to thrombin or any component of the formulation
- Do not inject intravascularly due to risk of severe systemic thrombosis and disseminated intravascular coagulation (DIC)
Adverse Reactions
- Allergic reactions including anaphylaxis with repeat exposure
- Development of antibodies against human or bovine thrombin
- Paradoxical hemorrhage due to consumption of clotting factors (systemic absorption)
Drug Interactions
- Topical products applied to the site may be inactivated by strong oxidizing agents or iodine
- Systemic procoagulant risk increases if co-administered with systemic hemostatic agents
