Forms & Strengths
- injection: 1000 mcg/mL
- oral: 50-1000 mcg
- nasal spray: 500 mcg/spray
Adult Dosing
- pernicious anemia: 100-1000 mcg IM daily
- vitamin B12 deficiency: 100-1000 mcg IM daily
- maintenance: 100-1000 mcg IM monthly
Pediatric Dosing
- children: 100-250 mcg IM daily
- infants: 50-100 mcg IM daily
- neonates: 10-30 mcg IM daily
Indications
- pernicious anemia
- vitamin B12 deficiency
- megaloblastic anemia
- increased homocysteine levels
Mechanism of Action
- necessary for DNA synthesis
- involved in fatty acid and amino acid metabolism
- plays a role in the metabolism of homocysteine
- necessary for the production of red blood cells
Contraindications
- hypersensitivity to cyanocobalamin or any component
- hereditary optic nerve atrophy
- Leber's disease
Adverse Reactions
- diarrhea
- itching
- rash
- hives
- anaphylaxis
Drug Interactions
- chloramphenicol: may decrease the therapeutic effect of cyanocobalamin
- colchicine: may decrease the absorption of cyanocobalamin
- H2 blockers: may decrease the absorption of cyanocobalamin
- oral contraceptives: may increase the requirements for cyanocobalamin
