Forms & Strengths
- lyophilized powder for injection 310mg/vial
Adult Dosing
- initial dose 1mg/kg/day IV for 4 days, may increase to 2mg/kg/day
- maximum dose: 6mg/kg/day
Pediatric Dosing
- 1-2mg/kg/day IV, adjust according to clinical response
Indications
- acute intermittent porphyria
- other porphyrias (e.g. variegate porphyria, hereditary coproporphyria)
Mechanism of Action
- reduces synthesis of toxic porphyrin precursors
- inhibits aminolevulinic acid synthase (ALAS)
Contraindications
- known hypersensitivity to hemin
Adverse Reactions
- infusion site reactions
- renal impairment
- hypoesthesia
- hemolysis
Drug Interactions
- estrogen-containing oral contraceptives: may exacerbate porphyria symptoms
- other porphyrinogenic agents: may increase risk of adverse reactions
