Forms & Strengths
- Injection: 0.25mg, 0.5mg, 1mg
- Tablet: 0.25mg
Adult Dosing
- 0.1-1.5mg IV/IM daily
- Oral dosing varies by indication and patient response
- Loading dose: 0.5-1mg IV, maintenance: 0.1-0.5mg IV/IM
Pediatric Dosing
- IV/IM: 0.01-0.03mg/kg/day, adjusted according to response
- Infants and children oral dose varies, not well established
Indications
- Edema associated with congestive heart failure
- Atrial fibrillation
- Atrial flutter
- Paroxysmal tachycardia
Mechanism of Action
- Inhibits the sodium-potassium ATPase pump
- Increases intracellular calcium in cardiac muscle
- Enhances cardiac contractility
- Slows conduction through the AV node
Contraindications
- Digitalis toxicity
- Hypersensitivity to digitalis glycosides
- Severe sinus bradycardia
- Second- or third-degree AV block
Adverse Reactions
- Nausea and vomiting
- Fatigue
- Confusion
- Visual disturbances
- Arrhythmias
Drug Interactions
- Quinidine, amiodarone, and verapamil increase digitalis toxicity risk
- Thiazide diuretics, furosemide increase hypokalemia risk
- Beta blockers, calcium channel blockers can increase bradycardia risk
