Forms & Strengths
- Capsules: 400 mg, 600 mg
- Tablets: 400 mg, 600 mg
- Injections: 50 mg/mL
Adult Dosing
- Initial dose: 400 mg orally every 8 hours
- Maintenance dose: 600 mg orally every 8 hours
- Maximum dose: 1200 mg/day
Pediatric Dosing
- Not established for children under 16 years of age
- Use with caution in pediatric patients
Indications
- Treatment of life-threatening ventricular arrhythmias
- Treatment of ventricular tachycardia
Mechanism of Action
- Class Ib antiarrhythmic agent
- Blocks sodium channels
- Decreases automaticity of the heart
Contraindications
- Hypersensitivity to tocainide
- Second- or third-degree AV block
- Sick sinus syndrome
Adverse Reactions
- Numbness or tingling in hands and feet
- Dizziness or lightheadedness
- Nausea and vomiting
- Anorexia
- Rash
- Blood dyscrasias
Drug Interactions
- Concomitant use with other antiarrhythmic agents may increase risk of additive toxic effects
- May interact with lidocaine, quinidine, and phenytoin
- Concomitant use with beta blockers may increase risk of AV block
