Forms & Strengths
- Tablets: 50mg, 100mg, 150mg
- Intravenous solution: 10mg per 5mL
Adult Dosing
- Paroxysmal supraventricular tachycardia: 50-100mg orally every 12 hours
- Paroxysmal atrial fibrillation: 50-150mg orally every 12 hours
- Maximum dose: 400mg per day orally, 600mg per day IV
Pediatric Dosing
- Children > 1 year: 1-3mg/kg orally every 12 hours, not exceeding 100mg per dose
- Children < 1 year, neonates: dose adjusted based on clinical response
Indications
- Paroxysmal supraventricular tachycardia
- Paroxysmal atrial fibrillation
- Atrial flutter
Mechanism of Action
- Class IC anti-arrhythmic agent
- Blocks sodium channels, slowing cardiac conduction
- Prolongs cardiac action potential
Contraindications
- Pre-existing 2nd or 3rd degree AV block
- Cardiogenic shock
- Severe conduction system disease
Adverse Reactions
- Dizziness, headache, nausea, fatigue
- Vision disturbances, dyspnea
- Cardiac effects: conduction disturbances, heart block, arrhythmias
Drug Interactions
- Amiodarone: increased risk of cardiac arrhythmias
- Digoxin: increased risk of digitalis toxicity
- Beta blockers: additive negative inotropic effects
