Forms & Strengths
- Injectable solution: 50 mg/mL
- Intravenous injection
Adult Dosing
- Heart failure / Acute cardiac failure: 50 to 100 mg IV as a single dose or repeated as needed based on clinical response
- Max daily dose: As directed by hemodynamic monitoring and clinical status
Pediatric Dosing
- Safety and efficacy in pediatric patients not well established
- Use restricted to specialized pediatric cardiology settings when clinically indicated
Indications
- Acute heart failure
- Low cardiac output states
- Support of myocardial contractility
Mechanism of Action
- Cyclic adenosine monophosphate (cAMP) analog
- Acts as a positive inotrope by stimulating myocardial contractility
- Causes peripheral vasodilation, reducing afterload
Contraindications
- Hypersensitivity to bucladesine or related compounds
- Severe obstructive cardiomyopathy
- Ventricular tachyarrhythmias
Adverse Reactions
- Hypotension due to vasodilation
- Tachycardia and arrhythmias
- Nausea and vomiting
- Injection site reactions
Drug Interactions
- Other positive inotropes (additive cardiac stimulation)
- Antihypertensive agents (enhanced blood pressure lowering effects)
- Arrhythmogenic drugs (increased risk of ventricular arrhythmias)
