Forms & Strengths
- Tablets: 25 mg, 50 mg, 75 mg, 100 mg
- Injection: 50 mg/10 mL (5 mg/mL) solution for IV use
Adult Dosing
- Heart failure: 25 mg orally twice daily, titrate to 50 mg, then 75 mg twice daily as tolerated
- Acute heart failure: 50 mg IV bolus, followed by 24-72 hours infusion at 6.25-12.5 mcg/kg/hour
Pediatric Dosing
- Safety and efficacy not established
Indications
- Treatment of heart failure (NYHA Class II-IV) to reduce risk of hospitalization and cardiovascular death
- Treatment of acute heart failure to improve symptoms
Mechanism of Action
- Selective cardiac myosin activator, increases cardiac contractility without significant effects on heart rate or myocardial oxygen demand
Contraindications
- Hypersensitivity to omecamtiv mecarbil or any component of the product
Adverse Reactions
- Cardiac ischemic events, including myocardial infarction
- Heart failure exacerbation
- Atrial fibrillation
- Symptomatic bradycardia
- Hypotension
Drug Interactions
- May increase risk of adverse cardiac effects when used with other cardiac myosin activators, or with beta-blockers at high doses
- Monitor for potential increased exposure when used with strong CYP2C8 inhibitors (e.g., gemfibrozil)
