Forms & Strengths
- Sublingual tablets: 5 mg
- Oral tablets: 10 mg
Adult Dosing
- Angina pectoris prophylaxis: 5 to 10 mg orally 3 times daily
- Acute angina attack: 5 mg sublingually as needed
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Indications
- Prophylaxis and treatment of angina pectoris
Mechanism of Action
- Acts as an organic nitrate and nitric oxide donor
- Relaxes vascular smooth muscle predominantly in systemic veins and arteries
- Decreases myocardial oxygen demand by reducing preload and afterload
Contraindications
- Hypersensitivity to organic nitrates
- Concomitant use with phosphodiesterase-5 inhibitors (e.g., sildenafil)
- Severe hypotension or uncorrected hypovolemia
- Increased intracranial pressure (e.g., head trauma, cerebral hemorrhage)
- Angle-closure glaucoma
Adverse Reactions
- Throbbing headache
- Dizziness and orthostatic hypotension
- Flushing and reflex tachycardia
- Nausea and vomiting
Drug Interactions
- Severe profound hypotension with phosphodiesterase-5 inhibitors
- Additive hypotensive effects with antihypertensive agents and alcohol
- Potential decreased efficacy with concurrent use of non-steroidal anti-inflammatory drugs
