Forms & Strengths
- Extended-release tablets: 2.5mg, 5mg, 10mg
- Sublingual tablets: 2.5mg, 5mg, 10mg
- Transdermal patches: 5mg/24hr, 10mg/24hr
Adult Dosing
- Angina pectoris: 2.5-10mg sublingually, 5-10mg orally, 5-10mg/24hr transdermally
- Acute coronary syndrome: 2.5-10mg sublingually, 5-10mg orally
Pediatric Dosing
- Safety and efficacy not established for patients under 18 years
- Dosage has not been studied in pediatric patients
Indications
- Treatment of angina pectoris due to coronary artery disease
- Prophylactic management of angina pectoris
Mechanism of Action
- Relaxation of vascular smooth muscle
- Dilation of peripheral arteries and veins, especially coronary arteries
- Decreased myocardial oxygen demand
Contraindications
- Concomitant use of phosphodiesterase inhibitors (e.g., sildenafil)
- Concomitant use of riociguat
- Hypersensitivity to nitrates or any component of the formulation
- Increased intracranial pressure
Adverse Reactions
- Headache, dizziness, lightheadedness
- Hypotension, orthostatic hypotension
- Nausea, vomiting, diarrhea
Drug Interactions
- Phosphodiesterase inhibitors (e.g., sildenafil): increased risk of hypotension
- Riociguat: increased risk of hypotension
- Aspirin: increased risk of bleeding
- Blood pressure-lowering medications: increased risk of hypotension
