Adult Dosing

Indications

Mechanism of Action

Contraindications
- Hypersensitivity to organic nitrates, psycholeptics, or any component of the formulation
- Concurrent use with phosphodiesterase type 5 (PDE5) inhibitors (e.g., sildenafil) due to severe, potentially fatal hypotension
- Concurrent use with soluble guanylate cyclase (sGC) stimulators (e.g., riociguat)
- Severe hypotension, hypovolemia, increased intracranial pressure, or closed-angle glaucoma
Adverse Reactions
- Severe systemic hypotension, orthostatic dizziness, and reflex tachycardia
- Throbbing headaches, flushing, and transient syncopal episodes
- Central nervous system depression, excessive sedation, somnolence, and cognitive impairment from the psycholeptic component
- Paradoxical reactions, dependence, or withdrawal symptoms with prolonged psycholeptic use
Drug Interactions
- Phosphodiesterase-5 (PDE5) inhibitors: profound and dangerous hypotensive synergy
- Soluble guanylate cyclase (sGC) stimulators: contraindicated due to risk of severe hypotension
- Alcohol and other central nervous system depressants: enhanced sedative and hypotensive effects
- Antihypertensive agents and vasodilators: additive blood pressure-lowering effects
