Forms & Strengths
- 1mg/vial powder for injection
- 5mg/5mL solution for injection
- 6mg/6mL solution for injection
- 24.5mg/24.5mL solution for injection
Adult Dosing
- Initial bolus 2mcg/kg IV
- Continuous infusion 0.01-0.03mcg/kg/min
- Titrate to clinical response
Pediatric Dosing
- Limited pediatric data
- Use with caution in pediatric patients
- Dosing not established for patients under 18
Indications
- Acute decompensated congestive heart failure
- Short-term treatment of heart failure
- Reduction of symptoms of dyspnea
Mechanism of Action
- Human B-type natriuretic peptide (BNP) analogue
- Vasodilatory effects
- Decreases pulmonary capillary wedge pressure
- Increases urinary sodium excretion
Contraindications
- Hypersensitivity to nesiritide
- Low cardiac filling pressure
- Cardiogenic shock secondary to severe left ventricular dysfunction
Adverse Reactions
- Hypotension
- Dizziness
- Nausea
- Headache
- Renal impairment
Drug Interactions
- Caution with ACE inhibitors and ARBs
- Monitor blood pressure with concomitant vasodilators
- Potential for increased hypotension with beta-blockers
