Forms & Strengths
- Powder and solvent for solution for infusion: 5 g vial
Adult Dosing
- Severe ischemia/acute myocardial infarction: 70 mg/kg to 250 mg/kg IV once or twice daily
- Administer as a slow intravenous infusion (approximately 10 mL/min)
Pediatric Dosing
- Safety and efficacy in pediatric patients not well established; use generally restricted or off-label
- Severe hypoxia/ischemia: 250 mg/kg IV once daily as slow infusion
Indications
- Adjunctive treatment in acute myocardial infarction
- Cellular energy depletion states and severe tissue ischemia
Mechanism of Action
- Acts as an intermediate in cellular glycolysis, promoting intracellular ATP synthesis
- Enhances potassium uptake into cells and stabilizes erythrocyte membrane integrity
- Exerts anti-ischemic and cytoprotective effects by restoring high-energy phosphate pools
Contraindications
- Known hypersensitivity to fructose-1,6-diphosphate
- Fructose intolerance (hereditary fructose intolerance due to aldolase B deficiency)
- Hyperphosphatemia and severe renal failure
Adverse Reactions
- Flushing and warm sensations during rapid infusion
- Pain, irritation, or phlebitis at the injection site
- Nausea and mild gastrointestinal discomfort
- Allergic reactions including rash and pruritus
Drug Interactions
- Incompatible with alkaline solutions and calcium-containing solutions for Y-site administration
- May alter the effects of insulin in diabetic patients due to shifts in glucose metabolism
