CARDIOVASCULAR SYSTEM › ANTIHYPERTENSIVES › ANTIHYPERTENSIVES AND DIURETICS IN COMBINATION › Rauwolfia alkaloids and diuretics in combination
combination of rauwolfia alkaloids and diuretics incl. other combinations
C02LA50
Forms & Strengths
- Oral tablets containing standardized rauwolfia serpentina whole root combined with various thiazide diuretics (e.g., chlorothiazide, hydrochlorothiazide) or loop diuretics in fixed-dose combinations.
Adult Dosing
- Hypertension: Initial dose typically 1 tablet daily, adjusted based on patient response and blood pressure control.
- Maintenance: Usually 1 to 2 tablets daily administered as a single dose or divided.
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established; use is generally not recommended.
Indications
- Management of essential hypertension, particularly in patients requiring multi-mechanism therapy combining sympathetic inhibition and volume depletion.
Mechanism of Action
- Rauwolfia alkaloids (e.g., reserpine) deplete catecholamines and serotonin in central and peripheral adrenergic neurons, reducing sympathetic tone, cardiac output, and peripheral vascular resistance.
- Diuretics inhibit renal sodium and water reabsorption (e.g., in the distal convoluted tubule), reducing extracellular fluid volume and blood pressure.
Contraindications
- Known hypersensitivity to rauwolfia alkaloids, thiazides, or sulfonamide-derived drugs.
- Active peptic ulcer disease or ulcerative colitis.
- History of mental depression, especially with suicidal tendencies.
- Anuria or severe renal impairment.
- Concurrent use of monoamine oxidase inhibitors (MAOIs).
Adverse Reactions
- Central Nervous System: Severe depression, drowsiness, fatigue, nightmares, parkinsonian syndrome.
- Gastrointestinal: Nausea, vomiting, anorexia, activation of peptic ulcer, diarrhea.
- Cardiovascular: Bradycardia, orthostatic hypotension, arrhythmias.
- Metabolic/Electrolyte: Hypokalemia, hyperuricemia, hyperglycemia, hypercalcemia (due to diuretic component).
Drug Interactions
- MAOIs: May cause severe CNS excitation and hypertension.
- Digitalis glycosides or antiarrhythmics: Rauwolfia-induced bradycardia combined with digitalis may precipitate arrhythmias.
- Lithium: Diuretics decrease renal lithium clearance, increasing the risk of lithium toxicity.
- CNS depressants (alcohol, barbiturates): Potentiates sedative effects.
