bendroflumethiazide
C03AA01
Forms & Strengths
- Oral tablets: 2.5 mg
- Oral tablets: 5 mg
Adult Dosing
- Hypertension: 2.5 mg to 5 mg orally once daily in the morning
- Edema: 5 mg to 20 mg orally once daily or intermittently
Pediatric Dosing
- Hypertension/Edema (Safety and efficacy not well established in children): Initial 0.05 mg/kg/day to 0.4 mg/kg/day orally in 1 to 2 divided doses
Indications
- Management of hypertension
- Adjunctive therapy for edema associated with congestive heart failure, hepatic cirrhosis, and corticosteroid/estrogen therapy
Mechanism of Action
- Inhibits sodium and chloride reabsorption in the distal convoluted tubules of the nephron
- Increases excretion of sodium, chloride, water, and potassium, leading to reduced extracellular fluid volume and decreased blood pressure
Contraindications
- Anuria
- Hypersensitivity to bendroflumethiazide or other sulfonamide-derived drugs
- Severe hepatic coma or pre-coma
Adverse Reactions
- Hypokalemia, hyponatremia, hypomagnesemia, hypercalcemia
- Hyperuricemia and precipitating gout
- Hyperglycemia and dyslipidemia
- Orthostatic hypotension, dizziness, vertigo
- Photosensitivity and skin rashes
Drug Interactions
- Lithium: Decreased renal clearance, increasing risk of lithium toxicity
- Antihypertensive agents: Synergistic blood pressure lowering effects
- NSAIDs: May attenuate the diuretic, natriuretic, and antihypertensive effects
- Corticosteroids or ACTH: Increased risk of severe hypokalemia
- Digitalis glycosides: Hypokalemia increases risk of digitalis toxicity
