cyclopenthiazide
C03AA07
Forms & Strengths
- Tablets: 0.25 mg, 0.5 mg, 1 mg
Adult Dosing
- Hypertension: 0.25 mg to 0.5 mg orally once daily in the morning.
- Edema: 0.5 mg to 1 mg orally daily or intermittently.
Pediatric Dosing
- Safety and efficacy in pediatric patients not established; use generally avoided or individualized based on clinical expert guidance.
Indications
- Management of hypertension
- Treatment of edema associated with congestive heart failure, hepatic cirrhosis, or corticosteroid/estrogen therapy
Mechanism of Action
- Thiazide diuretic that inhibits sodium and chloride reabsorption at the early distal convoluted tubule in the kidneys
- Increases excretion of sodium, chloride, water, and potassium, leading to reduced blood volume and decreased peripheral vascular resistance
Contraindications
- Anuria
- Hypersensitivity to thiazides or sulfonamide-derived drugs
- Severe renal or hepatic impairment
- Refractory hypokalemia, hyponatremia, or hypercalcemia
Adverse Reactions
- Hypokalemia, hyponatremia, hypomagnesemia, hypercalcemia
- Hyperuricemia and gout exacerbation
- Hyperglycemia and dyslipidemia
- Orthostatic hypotension, dizziness, and fatigue
- Photosensitivity and skin rashes
Drug Interactions
- Lithium: Decreased renal clearance, increasing risk of lithium toxicity
- Antihypertensive agents: Synergistic blood pressure reduction
- NSAIDs: May attenuate the diuretic and antihypertensive effects and increase renal impairment risk
- Corticosteroids and ACTH: Increased risk of severe hypokalemia
- Digitalis glycosides: Increased toxicity risk if thiazide-induced hypokalemia occurs
