Forms & Strengths
- Oral tablet: 15 mg
- Oral tablet: 25 mg
- Oral tablet: 50 mg
- Oral tablet: 100 mg
Adult Dosing
- Hypertension: 12.5 mg to 25 mg orally once daily; may increase up to 50 mg daily if needed
- Edema: 50 mg to 100 mg orally once daily, or 100 mg 3 times per week

- Management of edema associated with congestive heart failure, hepatic cirrhosis, or corticosteroid/estrogen therapy
Mechanism of Action
- Inhibits sodium and chloride reabsorption in the cortical diluting segment of the distal convoluted tubule
- Increases excretion of sodium, chloride, and water, while decreasing calcium excretion and increasing potassium and magnesium excretion
Contraindications
- Anuria
- Hypersensitivity to chlortalidone or other sulfonamide-derived drugs
- Severe electrolyte depletion states
Adverse Reactions
- Hypokalemia, hyponatremia, hypomagnesemia, hypercalcemia
- Hyperuricemia and gout exacerbation
- Hyperglycemia and impaired glucose tolerance
- Orthostatic hypotension, dizziness, and fatigue
- Photosensitivity and rash
Drug Interactions
- Lithium: Decreased renal clearance, increasing risk of lithium toxicity
- NSAIDs: May attenuate the antihypertensive and diuretic efficacy of chlortalidone
- Other antihypertensive agents: Synergistic blood pressure lowering effects
- Corticosteroids or ACTH: Increased risk of severe hypokalemia
