Forms & Strengths
- Injection: 20mg
- Tablet: 50mg, 100mg, 200mg
Adult Dosing
- Hypertension: 100-200mg orally daily
- Edema: 50-100mg orally daily
- Heart failure: 50-100mg orally daily
Pediatric Dosing
- Not recommended for pediatric patients under 18 years old
Indications
- Treatment of edema associated with congestive heart failure
- Treatment of hypertension
- Treatment of edema associated with nephrotic syndrome
Mechanism of Action
- Aldosterone antagonist
- Inhibits effects of aldosterone on the distal convoluted tubules
- Increases sodium excretion and water excretion
Contraindications
- Hyperkalemia
- Anuria
- Severe renal impairment
Adverse Reactions
- Gynecomastia
- Impotence
- Hyperkalemia
- Nausea
- Vomiting
Drug Interactions
- Potassium supplements: increased risk of hyperkalemia
- ACE inhibitors: increased risk of hyperkalemia
- NSAIDs: reduced diuretic effect
