Forms & Strengths
- Solution for injection: 1000 IU/mL
- Solution for injection: 5000 IU/mL
- Gel: 1000 IU/g
- Ointment: 1000 IU/g
Adult Dosing
- Prevention of DVT: 5000 IU s.c. 2 hours pre-op, then 5000 IU s.c. 8-12 hours post-op
- Treatment of DVT: 1000 IU/hour i.v.
- Topical application: apply a thin layer to affected area 2-3 times a day
Pediatric Dosing
- Not established for children under 18 years
- Use with caution in pediatric patients, adjust dose based on renal function and body weight
Indications
- Prevention of deep vein thrombosis (DVT)
- Treatment of DVT and pulmonary embolism
- Topical treatment of varicose veins and hemorrhoids
Mechanism of Action
- Inhibits thrombin and factor Xa, reducing blood coagulation
- Enhances activity of antithrombin III
Contraindications
- Hypersensitivity to heparin or heparinoids
- Active bleeding or bleeding disorder
- Severe renal impairment
Adverse Reactions
- Bleeding complications
- Hypersensitivity reactions
- Allergic dermatitis (topical application)
- Thrombocytopenia
Drug Interactions
- Increased bleeding risk with oral anticoagulants, aspirin, and NSAIDs
- Reduced effectiveness with platelet inhibitors
- Increased risk of hyperkalemia with potassium-sparing diuretics
