Forms & Strengths
- Metoprolol 50mg/Felodipine 5mg tablet
- Metoprolol 100mg/Felodipine 10mg tablet
- Extended-release tablets
Adult Dosing
- Initial dose: 1 tablet (Metoprolol 50mg/Felodipine 5mg) daily
- Titrate every 2 weeks as needed, up to 2 tablets (Metoprolol 100mg/Felodipine 10mg) daily
- Maximum dose: Metoprolol 200mg/Felodipine 20mg daily
Pediatric Dosing
- Not recommended for children under 18 years
- Safety and efficacy not established in pediatric patients
Indications
- Hypertension
- Angina pectoris
- Myocardial infarction
Mechanism of Action
- Metoprolol: beta-1 selective adrenergic receptor blocker, decreases heart rate and myocardial contractility
- Felodipine: calcium channel blocker, inhibits L-type calcium channels, relaxes vascular smooth muscle
Contraindications
- Hypersensitivity to metoprolol, felodipine, or other beta blockers and calcium channel blockers
- Sick sinus syndrome, unless a pacemaker is in place
- Cardiogenic shock
- Untreated pheochromocytoma
Adverse Reactions
- Fatigue, dizziness, headache
- Bradycardia, hypotension
- Nausea, abdominal pain, constipation
- Edema, flushing
Drug Interactions
- Verapamil and diltiazem: increased risk of AV block and hypotension
- Digoxin: increased risk of digitalis toxicity
- Clonidine: increased risk of hypotension and bradycardia
- CYP2D6 and CYP3A4 inhibitors: potential increased metoprolol levels
