Forms & Strengths
- Tablets: 2.5mg/5mg, 5mg/5mg, 5mg/10mg, 10mg/5mg, 10mg/10mg
- Fixed-dose combination of bisoprolol and amlodipine
Adult Dosing
- Initial dose: 2.5mg/5mg once daily
- Titrate every 7-14 days, maximum dose: 10mg/10mg once daily
- Dose adjustment for renal impairment or hepatic impairment may be necessary
Pediatric Dosing
- Not recommended for pediatric patients under 18 years
- Limited data available for pediatric use
Indications
- Hypertension, alone or in combination with other antihypertensive agents
- Angina pectoris, stable and vasospastic
- Myocardial infarction, early treatment and long-term management
Mechanism of Action
- Bisoprolol: beta-1 selective adrenergic receptor blocker, reduces heart rate and myocardial contractility
- Amlodipine: calcium channel blocker, inhibits L-type calcium channels, dilates peripheral arteries
Contraindications
- Hypersensitivity to bisoprolol, amlodipine, or any component of the formulation
- Sinus bradycardia, second- or third-degree AV block, cardiogenic shock
- Severe hepatic impairment, acute myocardial infarction with heart failure
Adverse Reactions
- Headache, dizziness, fatigue, nausea, peripheral edema
- Bradycardia, hypotension, bronchospasm, worsening of heart failure
Drug Interactions
- Negative inotropes (e.g., beta-blockers, calcium channel blockers), increased risk of hypotension and AV block
- CYP3A4 inhibitors (e.g., ketoconazole, ritonavir), increased amlodipine exposure
- CYP2D6 inhibitors (e.g., fluoxetine, paroxetine), increased bisoprolol exposure
