levamlodipine
C08CA17
Forms & Strengths
- Oral tablets: 2.5 mg
- Oral tablets: 5 mg
Adult Dosing
- Hypertension: Initial 2.5 mg once daily, titrated up to 5 mg once daily based on response.
- Coronary Artery Disease: 2.5 mg to 5 mg once daily.
Pediatric Dosing
- Hypertension (pediatric patients 6 years and older): 2.5 mg once daily.
Indications
- Treatment of hypertension to lower blood pressure.
- Treatment of coronary artery disease, including chronic stable angina and vasospastic angina.
Mechanism of Action
- Chiral isomer (S-amlodipine) of amlodipine, a dihydropyridine calcium channel blocker.
- Inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle.
- Causes peripheral arterial vasodilation, reducing total peripheral resistance and blood pressure.
Contraindications
- Known hypersensitivity to amlodipine or other dihydropyridines.
- Severe hypotension.
Adverse Reactions
- Peripheral edema
- Dizziness
- Flushing
- Fatigue
- Palpitations
Drug Interactions
- CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) may increase plasma concentrations of levamlodipine.
- CYP3A4 inducers (e.g., rifampin, St. John's wort) may decrease plasma concentrations.
- Simvastatin exposure may be increased when co-administered.
- Blood pressure-lowering effects may be enhanced by concurrent antihypertensive medications.
