Forms & Strengths
- Tablets: 6.25mg, 12.5mg, 25mg
- Oral solution: 5mg/5mL
Adult Dosing
- Hypertension: 6.25-12.5mg once daily, titrate to 25-50mg once daily
- Heart failure: 6.25-12.5mg once daily, titrate to 25mg once daily
Pediatric Dosing
- Not established for children under 18 years
- Use with caution in pediatric patients with renal impairment
Indications
- Hypertension
- Chronic heart failure
- Left ventricular dysfunction
Mechanism of Action
- Inhibits angiotensin-converting enzyme (ACE)
- Reduces angiotensin II and aldosterone levels
- Increases bradykinin levels, causing vasodilation
Contraindications
- Hypersensitivity to spirapril or other ACE inhibitors
- Angioedema related to ACE inhibitors
- Bilateral renal artery stenosis
- Renal failure (creatinine clearance <30mL/min)
Adverse Reactions
- Cough (11-16%)
- Dizziness (4-8%)
- Headache (4-8%)
- Hypotension (3-7%)
- Hyperkalemia (2-5%)
- Renal dysfunction (2-5%)
Drug Interactions
- Potassium-sparing diuretics: increased risk of hyperkalemia
- Potassium supplements: increased risk of hyperkalemia
- Lithium: increased lithium levels, monitor carefully
- NSAIDs: reduced antihypertensive effect, increased risk of renal dysfunction
