Forms & Strengths
- Tablets: 2.5mg, 5mg, 10mg, 20mg
- Combination with hydrochlorothiazide: 10mg/25mg
Adult Dosing
- Initial dose: 2.5-5mg once daily, titrate up to 40mg
- Usual dose: 10-20mg once daily
- Maximum dose: 40mg once daily
Pediatric Dosing
- Initial dose: 0.1mg/kg (5mg/m2) once daily, titrate up to 0.5mg/kg (20mg/m2)
- Children >6 years: 5-10mg once daily
Indications
- Hypertension
- Heart failure
- Left ventricular dysfunction
- Diabetic nephropathy
Mechanism of Action
- Inhibits ACE, preventing conversion of angiotensin I to angiotensin II
- Increases bradykinin, prostaglandins, decreasing peripheral resistance
Contraindications
- Hypersensitivity to enalapril or diuretics
- Bilateral renal artery stenosis
- History of angioedema
- Pregnancy (2nd, 3rd trimester)
Adverse Reactions
- Cough
- Hypotension
- Hyperkalemia
- Dizziness
- Headache
- Fatigue
Drug Interactions
- Potassium-sparing diuretics: increased risk of hyperkalemia
- NSAIDs: reduced antihypertensive effect
- Lithium: increased risk of lithium toxicity
