Forms & Strengths
- Tablets: 5mg, 10mg, 20mg, 40mg quinapril
- Fixed-dose combination with hydrochlorothiazide: 10mg quinapril/12.5mg hydrochlorothiazide, 20mg quinapril/12.5mg hydrochlorothiazide, 20mg quinapril/25mg hydrochlorothiazide
Adult Dosing
- Hypertension: initial dose 10mg once daily, titrate to 20-80mg once daily
- Heart failure: initial dose 5mg twice daily, titrate to 20-40mg twice daily
Pediatric Dosing
- Not recommended for children under 18 years due to limited safety and efficacy data
- For pediatric patients with hypertension, consideration of other agents with established safety profiles is recommended
Indications
- Treatment of hypertension
- Treatment of heart failure
Mechanism of Action
- Quinapril: competitive inhibition of angiotensin-converting enzyme (ACE), reducing conversion of angiotensin I to angiotensin II, a potent vasoconstrictor
- Diuretic component (e.g. hydrochlorothiazide): inhibits sodium reabsorption in the distal convoluted tubule, increasing sodium and water excretion
Contraindications
- Hypersensitivity to quinapril, other ACE inhibitors, or diuretics
- History of angioedema related to ACE inhibitor therapy
- Hereditary or idiopathic angioedema
- Anuria or severe renal impairment

- Cough
- Dizziness
- Orthostatic hypotension
- Headache
- Fatigue
- Nausea
- Hyperkalemia
- Impaired renal function
Drug Interactions
- Potassium supplements or potassium-sparing diuretics: increased risk of hyperkalemia
- Lithium: increased lithium levels and risk of lithium toxicity
- NSAIDs: decreased ACE inhibitor effectiveness, increased risk of renal impairment
- Digoxin: increased digoxin levels
