Forms & Strengths
- Oral granules: 5 g per packet
- Oral granules: 5 g per scoop in bulk containers
- Oral tablets: 1 g
Adult Dosing
- Hyperlipidemia (Tablets): 2 g to 16 g orally once daily or in 2 divided doses
- Hyperlipidemia (Granules): 5 g to 30 g orally once daily or in 2 divided doses
- Initial dose: 2 g tablets once or twice daily, or 5 g granules once or twice daily
- Titration: Increase by 2 g (tablets) or 5 g (granules) at 1- to 2-month intervals
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Indications
- Adjunct to diet for the reduction of elevated serum total and LDL cholesterol in patients with primary hypercholesterolemia
- Pruritus associated with partial biliary obstruction
Mechanism of Action
- Bile acid sequestrant that binds bile acids in the intestine, forming an insoluble complex
- Prevents enterohepatic reabsorption of bile acids, leading to increased hepatic conversion of cholesterol to bile acids
- Upregulates hepatic LDL receptors, increasing clearance of LDL cholesterol from the blood
Contraindications
- Complete biliary obstruction
- Hypersensitivity to colestipol or any component of the formulation
Adverse Reactions
- Constipation
- Abdominal pain and distention
- Indigestion, nausea, and vomiting
- Flatulence, diarrhea, and bloating
- Headache and dizziness
- Hyperchloremic acidosis (rare, with long-term use)
Drug Interactions
- Reduces absorption of anionic and lipophilic drugs (e.g., digoxin, warfarin, thiazide diuretics, thyroid hormones)
- Administer other oral medications at least 1 hour before or 4 to 6 hours after colestipol
- May decrease absorption of fat-soluble vitamins (A, D, E, K)
