Forms & Strengths
- Tablets: 500mg, 750mg, 1000mg
- Extended-release tablets: 500mg, 750mg, 1000mg
- Sustained-release tablets: 250mg, 500mg, 750mg, 1000mg
Adult Dosing
- Hyperlipidemia: 500mg to 2000mg orally, once or twice daily
- Pellagra prophylaxis: 100mg to 500mg orally, once daily
- For combination therapy: follow specific product instructions
Pediatric Dosing
- Pellagra treatment: 50mg to 100mg orally, once daily, or as directed by a physician
- Hyperlipidemia: Safety and efficacy not established in pediatric patients
Indications
- Hyperlipidemia, types IIa and IIb
- Pellagra prophylaxis and treatment
- Peripheral artery disease and cerebrovascular accident prophylaxis
Mechanism of Action
- Nicotinic acid (niacin) inhibits lipolysis in adipose tissue, reducing free fatty acids in the bloodstream
- Inhibits hepatic synthesis of VLDL, thereby decreasing LDL and triglycerides
- Increases HDL levels
Contraindications
- Active peptic ulcer disease
- Severe hyperglycemia or diabetes mellitus
- Severe gout or liver dysfunction
Adverse Reactions
- Flushing and pruritus
- Gastrointestinal upset, including nausea, vomiting, and diarrhea
- Hyperglycemia, hyperuricemia, and hepatic enzyme elevations
Drug Interactions
- Increases the risk of myopathy with HMG-CoA reductase inhibitors
- May enhance the effects of antihypertensive and antidiabetic agents
- Aspirin may reduce the flushing associated with nicotinic acid
