Adult Dosing
- Dermatomycosis, candidiasis, tinea versicolor: Apply cream, gel, or suspension to affected and surrounding skin twice daily, morning and evening, for up to 4 weeks.
- Seborrheic dermatitis: Apply shampoo to wet hair, massage to scalp, leave on for 3 minutes, rinse, and repeat twice weekly for 4 weeks with at least 3 days between applications.
- Onychomycosis of fingernails and toenails: Apply 8% nail lacquer to all affected nails once daily (preferably at bedtime) over clean, dry nails. Remove previous coat with alcohol every 7 days.
Pediatric Dosing
- Tinea corporis, tinea cruris, tinea pedis, candidiasis, tinea versicolor (Children >= 10 years): Apply cream, gel, or suspension twice daily for up to 4 weeks.
- Seborrheic dermatitis (Safety and efficacy not established in children < 16 years).
- Onychomycosis (Safety and efficacy not established in children < 18 years).
Indications
- Topical treatment of tinea pedis, tinea cruris, and tinea corporis caused by Trichophyton rubrum, Trichophyton mentagrophytes, Epidermophyton floccosum, and Microsporum canis
- Topical treatment of cutaneous candidiasis caused by Candida albicans
- Topical treatment of tinea (pityriasis) versicolor caused by Malassezia furfur
- Topical treatment of seborrheic dermatitis of the scalp (shampoo)
- Topical treatment of immunocompetent patients with mild-to-moderate onychomycosis of fingernails and toenails without lunula involvement (nail lacquer)
Mechanism of Action
- Chelates polyvalent metal cations (Fe3+ and Al3+)
- Inhibits metal-dependent enzymes (such as cytochromes and peroxidases)
- Disrupts cellular metabolism, membrane transport, and macromolecular synthesis in fungal cells
Contraindications
- Hypersensitivity to ciclopirox or any component of the formulation
Adverse Reactions
- Pruritus
- Burning sensation at application site
- Erythema
- Worsening of clinical condition (in localized areas)
- Contact dermatitis
- Application site rash or dryness
- Nail disorders (periungual erythema, nail bed discoloration, or nail dystrophy with lacquer)
Drug Interactions
- No formal drug interaction studies have been performed with topical ciclopirox formulations.
- Systemic absorption is minimal; significant pharmacokinetic drug interactions are unlikely.
