Forms & Strengths
- Topical cream (combination antifungals/antibacterials/corticosteroids)
- Topical ointment
- Topical solution
Adult Dosing
- Apply thinly to affected area 1 to 2 times daily
- Limit duration typically to 2-4 weeks for combination steroid products to prevent skin atrophy
Pediatric Dosing
- Safety and efficacy not well established in infants and young children
- Use with caution, apply sparingly, and avoid occlusive dressings due to systemic absorption risk
Indications
- Tinea corporis, tinea cruris, and tinea pedis complicated by inflammation or secondary bacterial infection
- Cutaneous candidiasis
Mechanism of Action
- Combines an antifungal agent (e.g., azole or allylamine) to inhibit fungal cell wall synthesis
- Includes a corticosteroid for rapid anti-inflammatory, antipruritic, and vasoconstrictive effects
- May include an antibacterial agent to treat overlapping superficial bacterial infections
Contraindications
- Hypersensitivity to any active ingredient or excipient
- Viral skin infections (e.g., varicella, herpes simplex, vaccinia)
- Tuberculosis or fungal skin lesions without concomitant anti-infective therapy
- Rosacea and perioral dermatitis (for face application)
Adverse Reactions
- Skin irritation, burning, stinging, or dryness at application site
- Folliculitis, hypertrichosis, or acneiform eruptions
- Allergic contact dermatitis
- Systemic corticosteroid effects with prolonged use, large surface areas, or occlusion (hypothalamic-pituitary-adrenal axis suppression)
Drug Interactions
- Potent CYP3A4 inhibitors (when systemic absorption of azole components occurs)
- Additive immunosuppressive effects with concurrent systemic corticosteroids
- Potential interference with topical vaccines or local immune response
