combinations of corticosteroids
D07AB30
Forms & Strengths
- Topical creams, ointments, lotions, and solutions
- Combination formulations containing moderate-potency corticosteroids (e.g., hydrocortisone valerate, triamcinolone acetonide) with antimicrobials, antifungals, or keratolytics
Adult Dosing
- Apply a thin film to the affected area 1 to 2 times daily as directed
- Limit continuous use to 2 consecutive weeks to minimize systemic absorption and local skin atrophy
Pediatric Dosing
- Safety and efficacy not well established in infants and young children
- Use lowest effective dose for the shortest possible duration; avoid use in the diaper area or under occlusive dressings
Indications
- Inflammatory dermatoses responsive to topical corticosteroids complicated by, or at risk for, secondary bacterial or fungal infections
- Psoriasis, eczematous dermatitis, and contact dermatitis with secondary infection
Mechanism of Action
- Corticosteroid component binds to intracellular glucocorticoid receptors, suppressing inflammation through down-regulation of pro-inflammatory cytokines and inhibition of phospholipase A2
- Anti-infective or keratolytic combination components provide synergistic antimicrobial or peeling activity tailored to the specific dermatological condition
Contraindications
- Hypersensitivity to any component of the formulation
- Untreated viral (e.g., varicella, herpes simplex), tuberculous, or severe fungal skin infections
- Rosacea and perioral dermatitis
Adverse Reactions
- Local: skin atrophy, striae, telangiectasia, folliculitis, acneiform eruptions, hypertrichosis, and hypopigmentation
- Systemic (with prolonged use or occlusion): hypothalamic-pituitary-adrenal (HPA) axis suppression, Cushing's syndrome, hyperglycemia, and glucosuria
Drug Interactions
- Strong CYP3A4 inhibitors (e.g., ritonavir, ketoconazole) may increase systemic absorption and risk of HPA axis suppression
- Concomitant use with other topical irritants or keratolytics may enhance local skin irritation
