Adult Dosing
- Atopic dermatitis: 600 mg subcutaneously initially, then 300 mg every other week
- Asthma: 200 mg to 300 mg subcutaneously every other week
- Chronic rhinosinusitis with nasal polyps: 300 mg subcutaneously every other week
- Eosinophilic esophagitis: 300 mg subcutaneously weekly
- Prurigo nodularis: 600 mg subcutaneously initially, then 300 mg every other week
Pediatric Dosing
- Atopic dermatitis (aged 6 months to 5 years): Weight-based dosing every 4 weeks
- Atopic dermatitis (aged 6 to 17 years): Weight-based dosing (200 mg or 300 mg) every 2 to 4 weeks
- Asthma (aged 6 to 11 years): 100 mg or 300 mg subcutaneously every 2 to 4 weeks based on weight
- Asthma (aged 12 to 17 years): 200 mg or 300 mg subcutaneously every other week
- Eosinophilic esophagitis (aged 11 years and older, weighing >=40 kg): 300 mg weekly
Indications
- Moderate-to-severe atopic dermatitis in patients aged 6 months and older
- Add-on maintenance treatment of moderate-to-severe asthma aged 6 years and older
- Add-on maintenance treatment for chronic rhinosinusitis with nasal polyps (CRSwNP)
- Treatment of eosinophilic esophagitis in patients aged 11 years and older
- Treatment of prurigo nodularis in adults
Mechanism of Action
- Fully human monoclonal IgG4 antibody
- Inhibits interleukin-4 (IL-4) and interleukin-13 (IL-13) signaling
- Binds to the shared IL-4 receptor alpha subunit
- Suppresses downstream Th2 inflammatory pathways, reducing IgE, eotaxin-3, and LDH
Contraindications
- Known hypersensitivity to dupilumab or any excipients in the formulation
Adverse Reactions
- Injection site reactions
- Conjunctivitis and keratitis
- Nasopharyngitis
- Upper respiratory tract infections
- Oral herpes
- Arthralgia
- Transient eosinophilia
Drug Interactions
- Avoid concurrent administration of live vaccines during therapy
- Potential effect on CYP450 enzymes by systemic inflammation normalization; monitor patients taking CYP substrates with narrow therapeutic index
