nystatin, combinations
G01AA51
Forms & Strengths
- Vaginal tablets: Nystatin 100,000 units
- Vaginal creams/ointments: Nystatin combined with antibacterial agents (e.g., oxytetracycline, diiodohydroxyquin) or anti-inflammatory agents
- Vaginal suppositories containing nystatin combinations
Adult Dosing
- Vulvovaginal candidiasis: Insert 1 vaginal tablet (100,000 units) or applicatorful of cream high into the vagina once or twice daily for 14 days
- Mixed vaginal infections: Follow specific combination product labeling for duration and frequency (typically 7 to 14 days)
Pediatric Dosing
- Not recommended for use in pediatric patients for vaginal indications
- Safety and efficacy in post-menarchal adolescents follow adult guidelines if clinically indicated
Indications
- Local treatment of vulvovaginal candidiasis (moniliasis)
- Treatment of mixed vaginal infections involving Candida albicans and susceptible bacteria or trichomonads depending on the specific combination components
Mechanism of Action
- Nystatin binds to sterols (primarily ergosterol) in the cell membrane of sensitive fungi, causing a change in membrane permeability and leakage of intracellular components
- Combination components (e.g., antibacterials) provide concurrent activity against bacterial pathogens present in mixed vaginal infections
Contraindications
- Hypersensitivity to nystatin or any component of the specific combination product
- History of hypersensitivity to other ingredients in the formulation (e.g., specific antibiotics included in combination)
Adverse Reactions
- Local irritation, burning, or pruritus upon application
- Contact dermatitis
- Rare systemic allergic reactions
Drug Interactions
- Minimally absorbed systemically via the vaginal route, resulting in a low potential for systemic pharmacokinetic drug interactions
- May degrade latex contraceptives (condoms, diaphragms) due to oil or chemical bases in certain cream/ointment formulations
