Forms & Strengths
- Vaginal cream: sulfanilamide 15% + sulfathiazole 3.42% + sulfacetamide 2.86%
Adult Dosing
- Intravaginal: One applicatorful (approx. 5 g) twice daily for 2 to 4 weeks
- Continue treatment through menstrual periods if necessary
Pediatric Dosing
- Safety and efficacy in pediatric patients not established
Indications
- Treatment of vaginitis, vulvitis, and cervicitis caused by susceptible microorganisms
- Adjunctive treatment in various infectious conditions of the lower female genital tract
Mechanism of Action
- Competitive inhibition of bacterial para-aminobenzoic acid (PABA) utilization
- Inhibition of dihydropteroate synthase, preventing folic acid synthesis required for bacterial growth
Contraindications
- Known hypersensitivity to sulfonamides
- Severe renal impairment
- Hefner's disease (acute intermittent porphyria)
- Pregnancy near term and in newborns due to risk of kernicterus
Adverse Reactions
- Local irritation, burning, or stinging at the application site
- Allergic contact dermatitis
- Systemic absorption leading rarely to hypersensitivity reactions, skin rash, or blood dyscrasias
Drug Interactions
- Local anesthetics (e.g., procaine, tetracaine) containing PABA may antagonize the antibacterial effect of sulfonamides
- Concomitant use of other vaginal preparations is generally not recommended
