carbetocin
H01BB03
Forms & Strengths
- Solution for injection: 100 mcg/mL in 1 mL vial
Adult Dosing
- Prevention of postpartum hemorrhage: 100 mcg administered as a single IV bolus or IM injection
- Administered strictly after delivery of the infant and immediately after elective or urgent caesarean section under epidural or spinal anesthesia
Pediatric Dosing
- Not applicable (neonatal and pediatric population)
Indications
- Prevention of uterine atony and postpartum hemorrhage following elective or urgent caesarean section under epidural or spinal anesthesia
Mechanism of Action
- Long-acting synthetic analogue of oxytocin that binds to oxytocin receptors in the myometrium
- Stimulates rhythmic contractions of the uterus, increases frequency of existing contractions, and raises uterine tone
- Reduces postpartum blood loss by promoting effective uterine involution
Contraindications
- Known hypersensitivity to carbetocin, oxytocin, or any component of the formulation
- Use during pregnancy prior to delivery of the infant
- Use for the induction of labor
- History of cardiovascular disease, severe hepatic or renal disease
- Eclampsia and pre-eclampsia
Adverse Reactions
- Very common (>10%): Nausea, abdominal pain, vomiting, pruritus, feeling of warmth, hypotension, headache, tremor, tachycardia
- Common (1-10%): Metallic taste, chest pain, back pain, dizziness, anemia, chills, fever
- Rare (<1%): Dyspnea, anaphylactoid reactions
Drug Interactions
- Concomitant administration with oxytocin or other uterotonics may lead to excessive uterine stimulation and hypertonicity; not recommended simultaneously
- May enhance the hypertensive effects of ergot alkaloids (e.g., methylergometrine)
- Anesthetic agents (e.g., halothane, cyclopropane) may modify the cardiovascular effects of carbetocin, potentially causing hypotension or conduction abnormalities
