Forms & Strengths
- Tablets: 5mg, 10mg, 20mg, 30mg, 40mg, 50mg
- Liquid oral formulations
Adult Dosing
- Initial dose: 20-60mg daily in 2-3 divided doses for Graves' disease
- Maintenance dose: 5-20mg daily in 1-2 divided doses
- Dose adjustment based on thyroid function tests
Pediatric Dosing
- Children: initial dose 0.5-1.5mg/kg daily in 2-3 divided doses for Graves' disease
- Dose adjustment based on thyroid function tests and clinical response
Indications
- Treatment of hyperthyroidism in Graves' disease
- Treatment of toxic multinodular goiter
- Preparation for thyroid surgery or radioactive iodine therapy
Mechanism of Action
- Inhibits the synthesis of thyroid hormones by blocking the oxidation of iodine in the thyroid gland
- Inhibits the enzyme thyroid peroxidase (TPO)
- Reduces the production of triiodothyronine (T3) and thyroxine (T4)
Contraindications
- Hypersensitivity to thiamazole or other thionamides
- Severe liver disease or liver dysfunction
- Agranulocytosis or severe neutropenia
Adverse Reactions
- Agranulocytosis, neutropenia, thrombocytopenia
- Hepatitis, liver dysfunction, jaundice
- Rash, urticaria, itching, Stevens-Johnson syndrome
- Gastrointestinal upset, nausea, vomiting
Drug Interactions
- Increased risk of agranulocytosis with concomitant use of antineoplastic agents
- Reduced efficacy with concomitant use of sulfonylureas or other thionamides
- Increased risk of hypothyroidism with concomitant use of iodine-containing contrast media
