parathyroid gland extract
H05AA01
Forms & Strengths
- Injectable solution: 100 USP units/mL
Adult Dosing
- Hypocalcemia: 20 to 40 USP units (2 to 4 mL) intramuscularly or subcutaneously every 12 to 24 hours as indicated by serum calcium levels
Pediatric Dosing
- Safety and efficacy not established in pediatric patients; use generally avoided in favor of modern calcium and vitamin D analogs
Indications
- Acute hypocalcemic tetany
- Adjunctive management of hypoparathyroidism
Mechanism of Action
- Directly stimulates osteoclast activity to mobilize calcium from bone
- Increases renal tubular reabsorption of calcium and decreases tubular reabsorption of phosphate
- Enhances gastrointestinal absorption of calcium indirectly via vitamin D activation
Contraindications
- Known hypersensitivity to parathyroid extract or animal-derived proteins
- Hypercalcemia
- Pre-existing digitalis toxicity
Adverse Reactions
- Allergic reactions and anaphylaxis due to foreign animal protein
- Hypercalcemia (nausea, vomiting, lethargy, weakness)
- Tachyphylaxis with prolonged use due to neutralizing antibody formation
- Local injection site reactions
Drug Interactions
- Digitalis glycosides: increased risk of cardiac arrhythmias due to hypercalcemia
- Calcium supplements: potentiation of hypercalcemic effects
- Thiazide diuretics: increased risk of hypercalcemia secondary to decreased renal calcium excretion
