Adult Dosing
- 0.5-1 mcg/day for chronic kidney disease
- 0.25-0.5 mcg/day for osteoporosis
- Individualized dosing based on serum calcium and phosphate levels
Pediatric Dosing
- Not established for children under 1 year
- 0.01-0.1 mcg/kg/day for pediatric chronic kidney disease
- Dosage adjustments based on clinical response and calcium-phosphate product
Indications
- Treatment of secondary hyperparathyroidism in patients with chronic kidney disease
- Treatment of osteoporosis in patients with vitamin D insufficiency
- Management of hypocalcemia
Mechanism of Action
- Selective agonist of the vitamin D receptor
- Regulates calcium and phosphate metabolism
- Inhibits parathyroid hormone secretion
Contraindications
- Hypercalcemia
- Vitamin D toxicity
- Known hypersensitivity to calcifediol or any component of the formulation
Adverse Reactions
- Hypercalcemia
- Nausea and vomiting
- Abdominal cramps
- Muscle weakness
- Drowsiness
Drug Interactions
- Thiazide diuretics (enhanced hypercalcemic effect)
- Cholestyramine (decreased calcifediol absorption)
- Phenobarbital and phenytoin (increased calcifediol metabolism)
- Digitalis glycosides (increased risk of digitalis toxicity)
